What is a Fistula-in-Ano?
A fistula-in-ano is an abnormal tunnel or channel that forms between the inside of the anal canal and the skin surrounding the anus. It is almost always the result of a previous anal abscess that did not heal completely.
Unlike fissures, fistulas never heal on their own. The tract is lined with tissue that prevents natural closure, causing persistent discharge, pain, and recurrent infections. Surgical or laser treatment is the only permanent solution.

Classification
Types of Anal Fistula

Intersphincteric
Passes through the internal sphincter only. The most common type — about 70% of all fistulas.
Transsphincteric
Passes through both internal and external sphincters. Accounts for about 25% of cases.
Suprasphincteric

Extrasphincteric
Passes outside both sphincters entirely. Rare and most complex — often associated with Crohn's or trauma.
